Medallion Medical Care R.N. Case Manager Provides case management for a team of clients. Performs initial client assessments & directs, plans & initiates appropriate plan of care for patients. Work as a team w/ RN's, & LPN's to provide effective & quality home care services. Other duties include maintaining/ developing rapport w/ providers of services, ancillary personnel & the general public. Registered Nurse license & current CPR certification required.
